Four people died including a three-year-old girl on the N2 about 30km from Makhanda.
Eastern Cape transport department spokesperson Unathi Binqose said a VW Polo with six occupants overturned and hit a concrete slab.
He said the jaws of life were used to extract those trapped in the front seat.
“The deceased include the male driver, two adult females and a toddler.”
He said two other women were seriously injured, and one was airlifted to a hospital in Gqeberha.
The other was rushed to a hospital in Makhanda.
Binqose said road safety was an every day concern and not just for the Christmas and Easter weekends.
“We call on people to remain vigilant on the roads.”
